CUSTOMER PROPRIETARY NETWORK INFORMATION (CPNI)

This communication regarding CPNI is provided in accordance with Federal Communication Commission regulations that require telecommunications carriers to provide notice concerning a carrier’s use of CPNI. CPNI is the information that Buisana gathers about its customer that relates to the customer’s usage of Buisana’s telecommunications services, and that Buisana becomes aware of as a result of the customer-service provider relationship.

Buisana and its affiliates are committed to protecting the confidentiality of CPNI. Buisana may, from time to time, have the opportunity to offer you products and services that will better meet your needs by using service-related information associated with the services you have already purchased from Buisana. If you elect to let Buisana use confidential service-related information for this purpose, we may also share the information with our Buisana affiliates, agents, or other related entities for the limited purpose of providing you with the most efficient and beneficial communications services possible and only after confidentiality conditions are in place.

You have the right to restrict Buisana’s use of your CPNI for these purposes. Your decision to restrict our use of CPNI will not affect the service(s) you receive from Buisana. If you wish to restrict Buisana’s use of your CPNI, please notify Buisana, in writing, or email us at the address below within 30 days of receipt of this notice. Moreover, you may also contact us in writing at any time in the future to restrict the use of your information. If you elect not to restrict use of your CPNI, such authorization will become effective forty-five (45) days from the date you were informed of your CPNI rights and Buisana may use your CPNI solely for the purposes described in this notice.

You may change your decision at any time, but your decision will remain valid until you inform us otherwise.
